When I start the Spot Filter, the rectangle flashes and then disappears. It worked properly until a few weeks ago. I tried uninstalling (total uninstall) and reinstalling, it didn’t make a difference.
Works fine for me:
I’m on Windows 10 using Shotcut v23.11.29
Can you specify what is your operating system and version of Shotcut?
And if you can, make a video capture of the problem.

This happens by design when the play head is not over the selected clip. You should be actually viewing the clip that you intend to interact with when using an on-video visual control. Double-click a clip to select it and seek to it.
