This seems to be broken since 5 months now. Various versions.
if I do not manually type .mp4 it makes an unusable file with no extension.
Adding the “meta.preset.extension=mp4” line made no difference.
Multiple users in this thread have said that it works fine for them. Can you provide more details about your operating systems and Shotcut versions and steps to reproduce? Does the problem still happen if Shotcut is restarted before attempting an export with a new custom preset?

I just tried this with version 21.03.21 and it worked both with and without a restart of Shotcut.

I mean documentation with all the possible command line to include into the extension file
meta.preset.X.Y = 0 - it makes no export happen
meta.preset.X.Y = 1 - it makes export entire timeline videos
meta.preset.X.Y = 5 - it makes just sound be exported
… and so one …
I feel I express myself a bit better 
After restarting the PC. It now seems to work
I now have .mp4 extension automatically on a said preset
The preset still forgets few other things, For example Audio becomes “Average Bit Rate” Rather than CBR.
Small annoyance but I can live with it.
